Please make sure that ALL checks are either made out in the name of the ALYN Hospital OR in the name of the Friends of ALYN Office or Chapter from which you would like to receive your receipt.

In order to make sure that the participant whom you are sponsoring receives credit for your donation, please send your check DIRECTLY either to ALYN Hospital Wheels of Love office OR to the relevant Friends of ALYN Office or Chapter, with full participant information.

To all sponsors who are U.K. tax payers  please also include a Gift Aid Form.

N.B. RECEIPTS CAN ONLY BE MADE OUT TO THE SIGNATORY ON THE CHECK.
